Do I have to write my LSAC number or name on my personal statement? Can't find this info on LSAC.

I would say no....but check school instructions. Some schools (Penn I think) specifically state to type your name and LSAC # on all attached documents. Read through the instructions and you'll figure it out.

I would put my name and LSAC number on every page of my application package. What happens if your PS slips out of your application folder and lands on the floor with some other papers? You want them to instantly be able to identify it.

This. Fill out part of an application, then preview it. You'll see how they put everything in the header. I just mimicked that for everything I attached to my apps. It gives it a nice, streamlined look.
