1.) In the spring/fall of xxxx I was diagnosed with xxxx. As I believed my academic performance would be severely impaired whilst I was undergoing treatment for xxxx, I asked for and was granted a medical withdrawal. I completed treatment and returned to xxxx in xxxx.
2.) In xxxx, at the age of fourteen, I was convicted/charged with/ arrested for/inhaled etc. In xxxx, at the age of sixteen,....... Insert contrition. End addendum.
