Redshirt in what? Should probably mention the sport (even though I'm sure it's mentioned elsewhere in your application.)

Perhaps say something like "these flare-ups coincide with the terms in which my GPA fell below X.X." Otherwise it sounds like it might have only flared up twice for a week or two but you're trying to blame three solid years of bad grades on it. Make sure it's clear that every time your grades fell, you had a medical reason.

It really doesnt matter. You could rewrite it as "I was abducted by aliens during freshman orientation and they implanted knowledge stealing doodads in my frontal lobe and anything I learned over the next 4 semesters was beamed to the alien craft so they could understand earth and I could not retain any of it." The admissions officer would read your statement, look at your GPA, and if its above their cutoff you'd be considered. If below you'd be out.

I know this is typically the case which is why I originally questioned writing one, but I was told to write an addendum because I am a URM. Not sure if that makes a difference, but I figured why not just write one?
