PS Fine Points

So Vanderbilt and maybe a couple other schools want your name and LSAC # at the top of your PS. Is this considered standard, or should I leave them unlabeled unless asked for?

I've had my name, "Personal Statement," and LSAC acct number at the top of mine for all schools. It's simple and since some schools ask for it, saves you the chance of making the mistake.
