Personal Statement Header

what do you guys put on your personal statement header? do you simply write "Personal Statement......................Name, LSAC #"?

Yep.
Left Justify: Full name, centered: Personal Statement, right justify: LSAC# LXXXXXXXX
Number your pages!

damn... I sent all my ps's out with no header, no title... I titled my ds and my gpa addenda, so I just assumed they'd know what it was- Am i screwed?

I numbered everything that was 2 pages or more. Why not?

I think you are fine. They get thousands of these. They will know. In the future, probably better to label things, but certainly not a game-changer. Unless it was a school/app that indicated they wanted a certain title.. I did see a school that had that. Maybe you could review your apps and if they ask for something particular you can decide if it is worth revisiting...

no you're not screwed, I don't think the adcomm will hold it against you in anyway, it's obvious that it's your PS, what else could it be.
