gpa addendum?

I think you should totally submit a GPA addendum (especially since at one point, you had a 4.0!). I think that admissions committees will really take into consideration your problems and the fact that you had to travel a lot as a result of a family member's illness. However, I would re-word what you have to something a little more formal.
