Just go:

you may want to include hard numbers - eg, FA08 was a 2.x gpa, whereas FA09 was a 3.x gpa - if the difference is significant.

it might be a good idea to state that you'll provide medical records upon request (and get a copy to have on hand in case they ask).

also, i don't like the phrase "deal with." "cope with," "manage," etc may be better. but that's probably just me being picky.

Isn't it bad form to be redundant on the application? They can easily see those numbers on my transcript.
In that case, I agree with cherry. The addendum looks good, but scratch the first part.
