Top Law Schools

So I know that almost all schools require that you have between a personal statement between 2 and 4 pages, double spaced, and with 12 point font. My question is about the format of the rest of the statement. Should I list my name with my LSDAS account number or should I just have the statement and nothing else? Thanks for the help

Some schools specify they want your LSDAS account number and name in the statement, and some don't. Just to make it easier on myself, I put them in the header of all of my personal statements...
