Which schools should I have it sent to? I'm still waiting to hear back from quite a few schools. Should I send it to only those I have been accepted to so far? Or all the ones I'm applying to?

I'm not sure of the right answer here but I plan on sending it to all schools I'm still in the running for (pending or waitlist) prior to the deadline, just in case.

Can I go back later and add new schools?

Yes you can but be aware that each time you change the school list (you can add or remove) you have to wait for the FASFA to be processed which can take a few days. Watch out for the deadlines.
