Now that I can finally post pictures, I guess I can add some pictures to the thread. Sorry about the awful quality and glare.
Here's the packet from U of Illinois:

Chicago is the only one I'm admitted to that says anything about having one. I guess it's a big deal, too, because one has to submit 1) seat deposit, 2) final transcripts, and 3) a Glass Menagerie photo before classes start. It's pretty nice, because you get one on the first day of orientation so when you forget someone's name you can go look it up when you get home. Profs also use them to generate a Paper-Chase style seating chart of the class for cold calling... which is... less nice. UPenn's is pretty cool. It's a big accordion file with little tabs (Housing, Financial Aid, Public Interest, etc.) and a different little booklet for each tab. Just sent through the normal mail, received about a week after admittance.
