Not for RD. Last year they came in January. But there is some speculation this was caused by the newly implemented skype interviews. So some are hopeful it will be sooner this year.

I've been complete since late August. RD interviews started around November 21 last year.

You're not screwed if you're a great fit for the school. Not putting the hours is only one of many things they'll take into consideration. My guess is that if you fit the mold for uchicago, they will at least offer you an interview in which you will be able to tell them about your involvement in various activities. I wouldn't worry about this one aspect of your application.

Same boat. The hours per activity are listed elsewhere on my app and my resume is well-done so I'm hoping it doesn't matter too much. The advice someone gave me was to just sit tight, don't contact admissions about it, and nail the interview.

I'm in the same boat as well. Oh well. I reread those instructions twice and I still forgot to go back in and add that to my resume.

I accidentally left that off last cycle and was admitted.

It's obviously not ideal, but it's by no means a dealbreaker.
