Mine was in the packet that with the letter dated from August. If you call the Department tomorrow ( the number is on the website) they will tell you where to get the form, download it and fill it out ( it takes longer to download than to complete)

Edit: just realized you were asking about the part where I said to mail your application I'm before interviewing. When they schedule the interview, there is a 1 page form they send you to fill out and send in with a $350 check. This is what has to be submitted prior to the interview.

Not sure if I can pay by card for this first payment, but I guess I'll find out when I get the letter.

Not to freak you out, but I hand-delivered mine at the 1st dep't last Wednesday and got my interview date / swearing in ceremony in the mail today. So I just got my interview / swearing in date in the mail, after filing for 1st dep't last Wed. I was worried that a few C&F questions would hold me up, so I'm wondering if this means that they won't be an issue or it just means that they've received my app.

Has anyone else in the Second Department had problems with attaining codes from the online Orientation to the Profession course? I just watched all the videos and didn't hear them give any of the codes and am a little confused

Just as an update:
Documents sent in approximately 11/13ish/2014.
Sent in an addendum on 11/30/2014 notifying them I got a speeding ticket on thanksgiving (fuck you nys police)
Called on 12/9/2014. Was notified that my application was marked as complete and sent to an interviewer on 12/5/2014.
Received a post card in the mail today, scheduling an interview on 12/23/2014. The interview takes place during working hours and is a few blocks away from my work.

How do you track down traffic ticket info?
I was traveling out of state and I got a traffic ticket in 2011. It was in VA but I have no idea the county (there are ~95 counties).
