Asking for Letters of Recommendation

What's the best way to approach a professor about writing a letter of recommendation for the purpose of transferring? I would assume going to talk to them in person is most appropriate. Does anyone have any success stories with this they could share? Maybe I'm just overthinking it, but I'm having hard time finding a non-awkward way to go about asking.
