I did a state (Texas) civ pro and I found the best guide to be a practice guide -- in that case, O'Connor's Texas Civil Trials, which is a Texas-based publisher. They also have a Fed Civil Trials guide. This is from another guide, but here's what a typical O'Connor's looks like.
