From Columbia's site:
Dean’s appraisal(s)/Certification(s) from the Dean or administrative officer responsible for student affairs at any undergraduate, graduate, or professional institution(s) at which you matriculated toward a degree.

You'd be surprised. Give your undergrad registrar a call. They may have a way to do it online so all you have to do is fill out your portion of the form, sign it, scan it, and then upload it to their site and send it to them. That's how it was for my undergrad.

I thought it would be a total PITA as well but it took me all of 15 minutes to get done counting the phone call to my undergrad.

Is Fordham considered T1 or closer to Top 20? Class rank hasn't come out yet but from the last few years I think I will be around top 5-7%.

Yup, i just submitted today. 3.70 at rank mid-40s. Anywhere from top 5-10% of my class. Have a masters degree from columbia as well and solid field work. Anyone want to comment on chances?

So we're supposed to apply on LSAC using the only option available right? Even though it says "Visiting"?

Yes. There should be an option later in the application that indicates you're applying to transfer. But if you're confused, just call admissions. They're great and helpful.

Columbia's admission is really weird. They don't use the LSAC status checker.

I applied on June 18th.
I confirmed that they had received all of my materials on June 25th.
I didn't go "Complete" until July 8th.
Got admitted on July 10th.

Moral of the story: As soon as you apply, call every couple of days until you can confirm they've received everything. The undergrad certs were the last thing to arrive.

Do they want individual physical, paper copies of transcripts sent to their office? Or does the CAS report cover it? Their website isn't much help.

Their website is clear:

Applicants may submit their current law school transcripts either through CAS or directly from their school's registrar's office. If submitting law school transcripts through CAS, the applicant must also forward an official explanation of the current grading system. Please forward updates of your transcripts to the Office of Admissions as soon as they become available.
Application Fee ($85), payable to “Columbia University.” (Source: http://web.law.columbia.edu/admissions/ ... -checklist)

Both NYU and Columbia go completely overboard on the forms they require. Each requires that your undergrad AND law school complete a dean's certification form and mail it to the admission office. Fortunately, Columbia, unlike NYU, will accept transcripts submitted through LSAC. As mentioned, you also have to submit a statement (you can submit it directly via email) that contains documents about your law school's grading system. Of course, once you submit all this, it will take over a week for them to be able to verify any materials that we're mailed....real pain. It's probably a good idea to call every few days to check if they've received the materials.

Any thoughts on when decisions might start to be given?

Probably not until July 9th or 10th at earliest. I think the first batch last year came out on Thursday, July 10th.

Received a complete email last night (6/26) at around 8pm eastern. They are obviously working overtime to process the applications.

I'm also "complete" despite things supposedly missing.
