From Columbia's site:
Dean’s appraisal(s)/Certification(s) from the Dean or administrative officer responsible for student affairs at any undergraduate, graduate, or professional institution(s) at which you matriculated toward a degree
Sigh... I am in New York now and I graduate undergrad from California, this is going to be troublesome.
You'd be surprised. Give your undergrad registrar a call. They may have a way to do it online so all you have to do is fill out your portion of the form, sign it, scan it, and then upload it to their site and send it to them. That's how it was for my undergrad.
I thought it would be a total PITA as well but it took me all of 15 minutes to get done counting the phone call to my undergrad.
