Has anyone gotten their offer letter from Willkie?

Got my offer over a week ago by phone from one of the interviewers. He said that recruiting would follow up with an offer letter through email but I haven't heard anything since. Probably just being neurotic.

For firms that do not do offer dinners or offer events, how do we approach the "second look" process to get more information and get a better feel for the firm?

Ask recruiting if you can do a second look, I'd reckon.

How do you accept an offer? Do you sign a hard copy of the offer letter? Or do you just phone or email the legal recruiter to say I accept the offer?

I CB'd with Dechert late last week and received an offer early this week, so I would wager a few days is the general turnaround. Judging from my timetable, it is possible they have a Friday meeting to determine offers so I wouldn't stress until after Monday.

Has anyone received rejections from screening interviews from the following firms:

Paul Hastings
Kaye Scholer
Mayer Brown

Do these firms do snail mail rejections? Because I have not heard a peep from them.

do you need to wear a suit for second looks, or is business casual more appropriate?

I assume you're a dude, just based on the nature of this question. I'd suit up but lose the tie

which firms don't do offer dinners? (assuming NY; obviously you can't necessarily go to the offer dinner for a firm in another market).

So far I've only been invited to one offer dinner. When do these dinners normally happen?

Does anyone know the turnaround time for Milbank? I have a CB with them later this week.
Interviewed with them Monday, CB Wednesday. But recruiting told me it could be up to 2 weeks because they make decisions by committee and the committee only meets once a week (normally Monday or Tuesday). So, I'd say it depends more on the day you interview and how quick your interviewers get their evals in.

Correction above for clarity. CB w/Milbank on Monday, Offer on Wednesday afternoon.
