In response to why: a host of reasons. Hard work, luck, skills, client relationships (both existing and new). My guess is that's the combo why just about anyone makes it.

Not quite the right way to think about it. Especially given the economic cycles of the past several years it's less a matter of "competition" and more a matter of "standard" given my practice group.
