I just answered those questions in an email to "email@example.com" and also threw in my LSAC #, GPA, and LSAT score.
I honestly self-reported my parents' income, which would certainly disqualify me for an LSAC waiver, but also submitted my paystubs from a part-time job that I have as the basis for my request. So if you have reason to request a need based waiver, and failed at getting a merit based one from Georgetown, I suggest you give this a try!
