Wow, this is like the first post of this nature that people have not really called BS on. I guess we all just want it to be true so bad. I would admit you, assuming it was a joke. The only thing I can think is that these forms are all official and stuff, and you are meant to be serious and truthful, ect. I highly doubt anyone would be a big enough tool to make something out of it, but meh.
