GPA addendum?

Hello just a quick yes or no is fine...
Would it be worth it to use a GPA addendum in my situation?
Transferred from CC to private college and semester GPA's were 4.0, 3.5, 3.0 (addendum), 3.4...
for the 3.0 semester I had meningitis and one professor refused to work with me on catching up resulting in a C- and tanking my semester GPA. I also suffered slightly in my other classes that semester due to missing 2+ weeks.
Worth it or no
