question about application form and resume for one of the work experiences

i was laid off from my job at a large company a few months ago. now that same employer company has again hired me at the equivalent level/role as before and the new job starts after 10 days.
in the application form do i need to fill two different employments with the gap in between, or can i just show it as a continuous employment in one position?
can i submit the app now with the new job as a current job (even though it officially begins after 10 days)?
waiting 10 days may be too much for an already late application.
