No need for an addendum unless the app requires it.

I personally think that if your reason is "i took the first test without studying, or I got better because I studied" that you should not write the addendum.

I think they will understand that you studied for it, hence the grade improvement. And saying you went into the first test unprepared is just foolish and not a great look.
