So far, from the T14:

Yale- lol, never asked
Harvard- never asked- is that a thing?
Stanford- never asked- is that a thing?
Columbia- "Will contact if eligible" or something
Chicago- Got unsolicited today
NYU- Emailed, denied based on no solicitations, will contact if eligible or something
Penn- Emailed, denied based on no solicitations, will contact if eligible or something
Berkeley- Emailed, denied based on no solicitations
Michigan- Got in mail a few weeks ago
Duke- Got by email a few months ago with PT
Virginia- Got at law school fair 3 weeks ago
Cornell- Got over email
Georgetown- Solicited, over email
Northwestern- Got over email, unsolicited

He got Cornell though, and they appear selective with waivers.

Near-perfect LSAT and GPA

Today I got Chicago and NYU.

Just recently got UCLA and Virginia.

Previously had Penn, Duke, Texas, Cornell.


I'm hoping for a Columbia waiver. Holding off on submitting that application until I can be certain of not getting one/I get one.

No, at least not always. I was disappointed I missed out on the Penn fee waivers a month ago and then I got mine a day or two after most people on this thread

I got waivers from NYU and UCLA, among others. I'm considering soliciting others but I'm not sure if my economic situation will justify it.

Do I just email or do I need to go through some form of application process for waivers?

I asked Chicago for a fee waiver and I got one yesterday, but I'm like 99% sure based on this thread that I was going to get it anyway and the timing is just a coincidence (it says on their website they don't take solicitations). I got from NYU on down unsolicited with a 3.76/170 and Columbia is the only non-HYS T-14 that hasn't given me one.
