I took a class at a local CC during my last semester of college (Spring 2012). The CC class ended like a week later than my home institution I graduated with my bachelors with. Was wondering If this grade would count towards LSDAS purposes? The CC transcript just says Spring 2012/does not contain specific dates. The CC grade was not transferred over to my home college but was taken while I was enrolled during my last year of college.

Yes, send it in. The LSAC page pretty clearly says to send in all transcripts from undergraduate and graduate institutions. Whether it will get counted toward your cumulative GPA is another question.
