Borderline addenda questions

I was considering writing an addendum for three situations that I think could use some application explaining. I haven't seen an answer in a quick search of the forums, but if it has already been addressed, a link would be a fine anser too. Each would be short and to the point. But are these necessary? And would writing an addendum help me?

LSAT score jumped 8-12 points from one administration of the test to the next (I misbubbled responses on final section)

Undergraduate abandonment of honors thesis: on my transcript it says honors thesis 1 for fall and nothing (and a 5 credit hr load) for spring. (Catastrophic leg injury with 40 hr/wk rehab for 6 wks and crutches for 12wks + bad break up)

Undergraduate bad grade early in career. My only C was in a class that I worked harder in than any other and am more proud of the C than many As. C was still in top 50% of class for almost all majors of foreign major.

What does your collective wisdom say? All? none? Some?

Do NOT write an addendum for one grade, and unless your leg injury caused a massive drop in your GPA (which I can't imagine it did being as you only took five credits that semester), don't write about that either. You could write about your LSAT but I assume you have no proof/documentation of your mis-bubbling so why even point it out. Your new LSAT score demonstrates that your first was anomalous.
