Quick Addendum Question

Two of my perspective schools asked on the application whether my education had ever been interrupted for a term or more. I answered "Yes" as I took a medical withdrawal from my first university in November 2009. I have no credits for that term (non-punitive Ws) although it would take a careful reading of my Academic Summary Report to realize this. Do I need to attach an addendum to these two applications to explain that "Yes" answer? Can I get away with answering "No"? I do have a paragraph discussing the withdrawal in my Personal Statement - is that good enough?

You should provide a brief addendum for a withdrawal. You should not answer the question untruthfully.

Answer 'Yes', write 2 paragraphs on what happened, and be done with it. Not a big deal at all.
As a side note, is the medical withdrawal thematic to your PS? It seems like something that probably doesn't add anything to the PS (or hints at a PS that needs a new/tighter focus), and would be better relegated to the addendum.
