So I was ticketed last year for doing a U-Turn and for driving without a license. I checked the guilty box and paid both tickets but how do I write this more formally?

This is the C&F question:

Have you ever been arrested for, convicted of or accused of any violation of law as a juvenile or adult? (Include all criminal offenses, i.e., felonies and misdemeanors, infractions, traffic offenses, juvenile matters, and everything else except for parking violations, regardless of the final disposition.)

On ______, 2010, in ____ county, __state__, I was ticketed for making an illegal U-turn and driving without a license. I plead guilty and paid the tickets in accordance with local laws.*
