Work Evaluation Form - Addendum to App?

I am a paralegal at a boutique litigation firm and I just had my annual evaluation wherein I received a print-out of nine associates' and one partner's feedback. It was overwhelmingly positive and I'd really like to include it in my application - can I submit it as an addendum to my application, a different way, or should I not submit it at all? I am getting and LOR from the partner. Thanks for your insight.
