No addendum is needed. I had a similar situation from high school that had a bigger impact on my LSAC GPA and my cycle played out the way that my numbers would have indicated.

Your GPA is excellent. I would not write an addendum drawing attention to the weakest part of your transcript, especially if it occurred even before you graduated from high school.
