I'm pretty sure there is no formal procedure anywhere.

An email is probably fine, although I think it looks slightly better if you mail it through the postal service. It costs one stamp and maybe three extra days max, so not a big deal. Then you won't need to worry about the body/attachment issue.

For the most part, I e-mailed my withdrawals out. I didn't catch any flak for it. If you'd feel better mailing it out, do that, but I don't see anything wrong with an e-mail.
