For Questions 4 and 5, it asks us to "list" things like the academic honors received. But it also asks us to "list" other activities and employment. For these, do we explain in paragraph form or just list the place of employment and activities?

I made a table like the ones found on other applications that ask for work experience, etc.

Either bullets or a table are fine. I've heard both from people who have been admitted.
