Do All schools require a letter from your dean?

Do All schools require a letter from your dean?

Or do most only require the 2 LORs? This has me confused, help!

For most schools you only need a Dean's Certificate if you have gotten written-up, disciplined etc. There are a couple of schools (Stanford and now Yale I think?) that require them outright.
