Ruining your chances

I've been obsessively checking browsing LSN, and I've spotted quite a few people with very high numbers (3.9+, 175+) who nevertheless have had much worse cycles than their numbers would have predicted.

What are some things that might drastically bring down an otherwise strong applicant's chances of being admitted? Applying a day before the deadline? Using the wrong school name? A gimmicky PS? Terrible LORs?

bad PS (e.g. poor writing, topic, or format)
bad LOR
yield protection
LSAT retakes (probably more important for HYS)

I know someone with a 3.9+/175+ who only got into one T14 due to PS/LOR.

How did he know his LOR was bad? And did he write a poem for his PS?

How did he know his LOR was bad? And did he write a poem for his PS?
I once read on an admissions blog about someone who wrote a self-addressed rejection letter for their PS. I'm guessing the adcomms just mailed that back to him in a few weeks.
