The common app. form filled out that information for me and my undergrad institution was listed only once, despite having been interrupted by study abroad.

Bump.
Anyone else? Leave it off the form?

List your main university first, with dates showing the full span.

List your study abroad institution second, with appropriate dates.

In the end, they have transcripts- they won't care about how you filled out the form. I'm not too worried they won't like how I filled out my study abroad- I have a transcript from a US university, the name of the program was something else, and the university where I studied was yet another institution.
