Retake fee waiver request

Sorry if this has been answered before but I couldn't find it via search (and I killed a few braincells last night during the wait for scores....)

I took the LSAT in June 08 the first time, for a 169, than again this Oct for a 173. I plan to send out a bunch of emails today requesting fee waivers, and know the general format is to include GPA (3.9), LSAT score and LSAC number. My question is do I have to list both scores? Will it look bad if I dont? What kind of schools do you think would give me waivers?
