LSDAS GPA is all that really matters.

Like others have said, cumulative GPA is the one that counts

If it makes you feel better, you will probably be looked at differently than a candidate who has the cumulative GPA but no extenuating circumstances.

As noted schools only care about your LSDAS cumulative GPA. No you don't need to mention that your grades dropped in the last semester. Also writing an addendum for the earlier grades is pointless unless you have a valid excuse like serious illness, death in the family, etc.
