I didn't. It's on my ASR, they have the transcripts. I put in whatever that could fit.

I did pose this question to a few schools. The general recommendation was to do an addendum listing all schools attended, and to notate "see attached addendum". One exception from Loyola Chicago, said to list with available space and to add the rest in an addendum.
