I'm sure this has been asked before but what is the proper format for writing an addendum? Does it need a title or what? Thanks.

Title it so that it is clear what it is, i.e. "LSAT Addendum," or "GPA Addendum." I would also include your name and LSAC#, but that is something I do with all the attached documents I submit.
