Is Stanford the only School with a Unique LOR Form?

Is Stanford the only school among T-14 that has its own LOR form (with check boxes on intellectual qualifications, writing skills, oral skills, maturity), and all other schools have the same generic LOR form where it just asks for a letter of recommendation?

Stanford was the only one that I recall. The only T14s I didn't apply to were Yale and Northwestern. GG fee waiver.
