Addendum format for a non-criminal court case

Has anyone here had to write an addendum for being involved in a non-criminal court case - specifically, small-claims? If so, how did you format it? Somehow the paragraph format that one would normally use for GPA/LSAT addenda does not seem appropriate to me. I want to make it as easy to read as possible.
A summary of what the case was about: I passed out at a friend's house when I was 16, but the people who were present could not bring me back to consciousness. They were worried and called an ambulance. My parents were supposed to take care of the bills related to that incident. However, they apparently failed to pay the ambulance company. In the spring of 2007, I was surprised when I received a court summons, especially since the unpaid bill went to court in 1996 (as I learned much later). I appeared as required and paid the bill, since my parents refused to do so. Basically, they told me "it's your problem now." I also did not want further negative consequences on any of my records, credit or otherwise. Only the following year, when consulting with my university's student legal servies, did I find out that I could have done something to fight it. Technically, since the service was rendered when I was a minor, it actually was my parents' responsibility, not mine.
If I have a copy of the actual court summary that the clerk sent me, would it be appropriate to attach it along with the addenum? Or, would it just be redundant and overkill?
