personally, I would not waste the time explaining your retake. (If I was an adcom, I'd wonder if the symptoms of food poisoning was just too much stress, and therefore I would see your addendum as an attempt as an excuse.)

-I wouldn't tell the adcomm what to do
-You basically come out and tell them you've been gunning for the LSAT for years
-It sounds like you had a slightly upset stomach and were a little off your game on test day. In the world of LSAT addendums that's pretty weak
-This thing leaves me with the exact same impression I would have got just by looking at your LSAT record: you didn't quite get what you needed out of it so you took it again (and again). No big deal. They're gonna take your highest score anyway.

If you really want to do an LSAT addendum here you go:
I had a mild case of food poisoning that I believe affected my performance on the October LSAT. I re-took the test in December with no issues; I feel this is more representative of my abilities.

would it be better to just not mention the cancel then and only mention the circumstances regarding the 167? or no addendum at all?

and if a cancel-167-173 does not merit an addendum, what exactly does?

unless you break 174, don't addendum
If so, say nothing except:
I had food poisoning. Got a bad score. then I didn't - please look more at second score
