PS length / formatting

If I'm taking a first cut at a PS - are there any rules of thumb to follow as far as length/formatting? I think I saw somewhere 2 pages 11 pt font will work for most schools?

Is there a specific header or should I just launch into text?

Do most people have a title?

11 size, 2 pages double spaced is standard default length

header should include name, lsac number

no title

got it. 11pt font, 2 pages, double spaced, no title.

can Name and LSAC number go on one line? Assuming that does not need to be double spaced from the rest of the document.
