Personal Statement Header?

Hi can someone give me an example of the header that we should use on our personal statement? Thanks

I think you just need to put your name and LSAC number, and "PErsonal Statement". Use insert header in word so it doesnt take up valuable space.

Also include page numbers, in case any pages of your file get out of order. Consider including the name of the school as well.
