PS Heading

What is the normal heading?? Is it:

Personal Statement Last name 1
LSAC Account #: -------

Or...

Personal Statement
LSAC Account #:--------
Last name 1

I had in the headers, name left justified, and LSAC number, right justified. I then had PERSONAL STATEMENT centered, but not in the headers. It worked fine for me.

It doesn't matter, there is no one right way to do it.

Just have the words "Personal Statement," your name, and your LSAC number at the top of every page. Number the pages too if you like but the order of it doesn't matter.

This does not matter at all. I've only had one school require a name and date on every attachment sent online. I went with "Personal Statement" centered at the top. And then a header in the top right with your choice (name, lsac #, pg 2)
