LSAT addendum, comments please
During my first and second time sitting for the LSAT I was insufficiently prepared due to an anxiety issue that severly detracted from my ability to concentrate. After thoroughly resolving my anxiety issue, my third sitting for the exam reflected a more accurate representation of my abilities as I was able to put in the necessary amount of effort in preparation.

I don't mean to sound mean... but it honestly comes across as making excuses. I understand that you were nervous but I don't think that the ad comm will consider someone telling you that you looked nervous to somehow explain your performance. And do you really want to tell them that you have a habit of procrastinating? I think you might need to start over. Sorry if that came across too harshly.

Instead, say you weren't sufficiently prepared the first two times, and the third LSAT more closely reflects your abilities than the previous two.

So it should be pretty short then, just a couple of sentences.
Essentially saying that I didn't prepare first two times around, and having prepared the third time, it was much closer to my abilities.

Agree with the above posts.
It should be short.
Don't say you are a habitual procrastinator.
Saying that someone vocalizing their observation caused you to be nervous and perform poorly is not addendum-worthy. People might say you look nervous before a 1L exam or the bar or a trial, it might shake you up but you can't use that as a legitimate explanation for poor performance.

You should nix anything that lies within your realm of control.
Example:
laziness - BAD
Mother dying of cancer - GOOD (not in the sense that you want your mother to die, but in the sense that this is a valid reason for being distracted)
Anything that you have control over and you failed to take control shows a weakness on your part - even is 99.9% of the other people on the planet would have done what you did, the law school admissions people will be that 0.1% who criticizes you for it because theygenuinely enjoy making our lives hell.

should it literally be the following:
During my first and second time sitting for the LSAT I was not sufficiently prepared for the task. I had not spent an adequate amount of time studying, and as a result my scores reflected this fact. My third sitting for the test was significantly improved over both of these first two sittings because I had prepared for the exam.

If you have documented anxiety problems and can include a note, this is worth an explanation (see your doctor and explain if you haven't, even if he doesn't prescribe anything he might be able to write a note such as...."person has a history of anxiety problems which he/she is treating successfully through exercise and stress reduction techniques"
if not, this really isn't an excuse and shouldn't be an addendum unless required.
