It is very mean that Cornell sends their alumni mag in an envelope. I see an envelope from Cornell and think "Sweet, some new info on next year." Then I open it and see a magazine with Cornell basketball on the cover and feel toyed with.

Let's all start sending them mysterious, white envelopes of our own with our respective high school's alumni mag inside. That'll learn 'em. See how they like it. 8)

So I finally took a look at the health forms that are due by June 13, and am freaking out a little at how I put it off till now. :oops: Could somebody please pm me the pdf's of the two pages (Parts 3 and 4) that the doctor needs to fill out, since I plan to go see him and request my vaccination info, do the physical examination, and have him fill out those forms all in my appointment tomorrow. Thanks in advance!

Fill out your part, hit submit, and then print the PDFs. Get them filled out and mail them in. You don't need anyone to PM them to you.

I just thought that it might be better to not hit submit until I have all my vaccination info rather than leaving it incomplete. So that's why I was hoping somebody could please send me the pdf's so that I could do it all in one sitting with my doctor.

It says complete to the best of your ability before you submit. It does not say it has to be complete. Fill in what you can, submit it, and have your doctor fill in the rest, and sign that they filled out the immunizations (there is a spot for that in section 4). It shouldn't be a problem. You have to mail it anyway with additional info--so I doubt they consider the one you submit online a complete form.

Or, you can do what I did: Call your doctor(& past doctors if needed), and any place you've had shots and just ask them to fax you the records. They'll make you verify somehow (birthdate and address or something), and then they should send it to you.

Also, I would only be concerned about filling out the ones you NEED: MMR, TB, tetanus, etc. and if you've had the others, fill them in later (ex. I filled all those in, and submitted, and will fill in the typhoid, hep a, etc. once I get them faxed from the travel clinic).

Sorry, but, redownloading the file, saving it, figuring out how to take my name/bday off the top, and then cutting out those two pages to PM to you is way more figuring out & work than I have time to do at the moment. :)

I think we should plan a TLS meeting before school starts. Any thoughts on this? There are some great bars downtown, and I'm also willing to offer my place up to hang out as well. Ideas?

I'd be down for a meet up before school starts. Don't know the area, so I'll leave venue recommendations to others.

But speaking of drinking, anyone tried Ithaca Beer? I can't wait to sample some new microbrewery deliciousness.
