Do any apps actually specifically request a DS?

Or does it fall under the category of "any other information you feel would be helpful for the admissions committee" and you just tack it on as an additional attachment with your application?

It is optional at 4 of the 5 schools I'm applying to. The 5th school didn't say anything about it one way or the other.
